Vision Plan Providers

Vision plans often have different credentialing and substitute provider requirements than medical insurance plans. Review the general guidance below and verify requirements with the appropriate vision plan before providing services.

VSP	Allowed with limits. Up to 10 consecutive days may not require notice. 11–60 consecutive days generally require advance notice and approval. No more than 60 days of fill-in coverage in a calendar year.	VSP Provider Services
EyeMed	Credentialing usually required. EyeMed generally requires all providers to be enrolled and credentialed before providing services. Fill-in or locum tenens coverage is not typically permitted without prior approval.	EyeMed Provider Portal or Provider Relations
