Vision Plan Providers

Vision plans often have different credentialing and substitute provider requirements than medical insurance plans. Review the general guidance below and verify requirements with the appropriate vision plan before providing services.

EyeMed generally requires all providers to be enrolled and credentialed before providing services. Fill-in or locum tenens coverage is not typically permitted without prior approval.
